As a prerequisite to registration, FIU requires all students born after December 31, 1956 to present documented proof of immunity to measles (Rubeola) and German Measles (Rubella). This written document must be signed by a physician (M.D. or D.O.), a nurse practitioner or a registered nurse and stamped by the doctor‟s office. Click here to obtain an Immunization Document Form. Please mail or fax it to the University Health Services on either campus. For identification purposes, a Panther ID number must be included on this form. Phone calls from your physician are not acceptable.
ACCEPTABLE FORMS OF DOCUMENTATION (of proof of immunity)
The following documents are also acceptable proof of immunity provided that the dates are acceptable and the documents are signed and stamped by the Health Care Provider:
- Childhood immunization records
- Health Department records
- Laboratory test results demonstrating immunity
- Military service records
- School immunization records
